Records Requests
For clients needing copies of their records, please send that request in writing to our office. Please specify the type of information you are requesting — billing, notes, full file — and your relationship to the client. If you are not the client or guardian, you must provide an exchange of information form signed by the client or guardian. A fee for records can be found in your intake documents or requested at the same time as the records. No fee for billing information is assessed. If notarization is needed, please note there is a separate fee for this service.

There may be an additional fee for redaction, if necessary.
